    ABSTRACT

Cognitive perceptual disruption (CPD) is an emotion regulation mechanism described in intensive short-term dynamic psychotherapy (ISTDP) that distinguishes fragile patients from resistant ones. CPD functions as an involuntary avoidance-based mechanism that prevents engagement with distressing emotions, making its identification crucial for guiding treatment. Currently, CPD assessment relies solely on clinical psychodiagnostic evaluation, with no available psychometric instruments. This study introduces a cognitive perceptual disruption screening instrument (Copeds), a self-report measure designed to identify CPD and assist in treatment planning for emotion-focused therapies.

    Learning & Clinical Insight

This groundbreaking study may mark a turning point in how we assess Cognitive Perceptual Disruption (CPD)—the hallmark anxiety pathway of fragile patients in ISTDP. Until now, identifying CPD relied solely on skilled clinical psychodiagnostic evaluation.

But now we finally have a promising psychometric screening tool: the CoPeDS (Cognitive Perceptual Disruption Screening) instrument.

    19-item self-report scale
    91% sensitivity to fragile patients
    Strong internal consistency (α = 0.88)
    Built entirely on ISTDP metatheory

    Clinical Takeaway
CoPeDS offers a structured, efficient way to support psychodiagnostic clarity before therapy even begins—potentially improving treatment matching, pacing, and outcomes for emotion-focused therapies.

This is an exciting step toward more precise, accessible identification of CPD—not just for ISTDP, but for the wider community of affect-focused psychotherapy.
