    ABSTRACT

    Cognitive perceptual disruption (CPD) is an emotion regulation mechanism described in intensive short-term dynamic psychotherapy (ISTDP) that distinguishes fragile patients from resistant ones. CPD functions as an involuntary avoidance-based mechanism that prevents engagement with distressing emotions, making its identification crucial for guiding treatment. Currently, CPD assessment relies solely on clinical psychodiagnostic evaluation, with no available psychometric instruments. This study introduces a cognitive perceptual disruption screening instrument (Copeds), a self-report measure designed to identify CPD and assist in treatment planning for emotion-focused therapies.

    Learning Points & Clinical Application of the CoPeds Instrument

    A new tool for working with fragile patients in ISTDP


    Cognitive Perceptual Disruption (CPD) has long been recognized in ISTDP as a marker of fragility — but until now, it’s been impossible to measure directly. The CoPeds instrument changes that.

    Developed through clinical and statistical rigor, CoPeds is a 19-item self-report tool designed to help therapists detect CPD symptoms — such as visual distortion, dissociation, thought blocking, and sensory anomalies — that indicate low ego-adaptive capacity.


    Key Learning Points:

    • Fragility is now measurable. CoPeds offers a quantifiable indicator of whether a patient’s symptoms fall within the fragile spectrum.

    • High AUC (0.88) shows strong ability to differentiate fragile from non-fragile cases.

    • CPD is no longer an intuitive guess. We can now screen and document these symptoms to guide treatment.


    Clinical Application:

    Use CoPeds at intake or early in assessment to help determine whether a patient requires regulation of anxiety and defense restructuring — or if they are ready for higher-pressure interventions.

    It’s especially useful when:

    • Symptoms are subtle or mixed with resistance

    • Supervisees struggle to spot fragility

    • You’re teaching ISTDP psychodiagnostics in Core or Pre-Core

    CoPeds doesn’t replace clinical judgment — but it sharpens it. It creates a common reference point between therapist, supervisor, and trainee.
